The following form needs to be completed for each participant in addition to being registered with USA Softball by the team coach or team administrator. There is no cost to complete this, but it is authorizing coaches and players to participate in our USA Softball of Hawaii leagues through August 31, 2024. It will also be used to provide USA Softball of Hawaii with necessary contact information.
If you are filling this out for a minor, we have also included USA Softball's Parents' Code of Ethics in our efforts to promote softball as a safe, fun, & educational experience for all involved.
